Forms & Waivers
Before arriving at the kayak shop please ensure that everyone in your group has completed the required forms and waivers.
Tours, Lessons and Rentals
Each participant must complete the Liability Waiver. If the participant is under 19 years old, a parent or guardian must complete the form on their behalf.
Guides and instructors often take photos and videos to share, complete the Media Consent form to be included.
Summer Camp & Youth Skills
The parent or guardian of each participant must complete the Acknowledgement of Risk form, the Summer Camp Medical Form and Youth Media Consent. All forms must be completed before the first day of camp. The youth media consent form is optional but we appreciate your participation
Overnight Trips
If you are going on a guided overnight trip with us, the Medical Form must also be completed.
